Unravelling the Fate of Coronary Artery Disease in Patients Undergoing Valve Replacement for Severe Aortic Valve Stenosis

چکیده

Severe aortic valve stenosis is the most frequent pathology in western world and approximately 50% of these patients have concomitant coronary artery disease (CAD). Revascularization proximal obstructive CAD undergoing surgical replacement (SAVR) common practice considered appropriate. However, management with transcatheter implantation (TAVI) more controversial. Nevertheless, performing percutaneous intervention (PCI) significant (>70%) lesions a widely adopted strategy, but robust supporting scientific evidence missing. Some studies suggest that complex incomplete revascularization negatively impacts outcomes post-TAVI. As increasingly younger are TAVI, optimizing long-term will become important. Although PCI TAVI safe, no benefit on has been demonstrated, possibly due to an inadequate selection prognostically important for revascularization. A possible solution might be use physiological indices, their own limitations data needed support widespread adoption. In this review we provide overview current after (AVR) regarding population.

Coronary artery disease and outcomes of aortic valve replacement for severe aortic stenosis.

OBJECTIVES The study sought to contrast risk profiles and compare outcomes of patients with severe aortic stenosis (AS) and coronary artery disease (CAD) who underwent aortic valve replacement (AVR) and coronary artery bypass grafting (AS+CABG) with those of patients with isolated AS who underwent AVR alone. BACKGROUND In patients with severe AS, CAD is often an incidental finding with undera...

Risk of acute kidney injury in patients with severe aortic valve stenosis undergoing transcatheter valve replacement.

BACKGROUND Transcatheter aortic valve implantation (TAVI) for high-risk and inoperable patients with severe aortic stenosis is an emerging procedure in cardiovascular medicine. Little is known of the impact of TAVI on renal function. METHODS We analysed retrospectively renal baseline characteristics and outcome in 58 patients including 2 patients on chronic haemodialysis undergoing TAVI at ou...

Coronary Artery Disease and Symptomatic Severe Aortic Valve Stenosis: Clinical Outcomes after Transcatheter Aortic Valve Implantation

INTRODUCTION The impact of coronary artery disease (CAD) on outcomes after transcatheter aortic valve implantation (TAVI) has not been clarified. Furthermore, less is known about the indication and strategy of revascularization in these high risk patients. AIMS This study sought to determine the prevalence and prognostic impact of CAD in patients undergoing TAVI, and to assess the safety and ...

Does the coronary disease increase the hospital mortality in patients with aortic stenosis undergoing valve replacement?

OBJECTIVES With the increase in life expectancy occurred in recent decades, it has been noted the concomitant increase in the prevalence of aortic stenosis and degenerative disease of atherosclerotic coronary artery. This study aims to evaluate the influence of atherosclerotic coronary artery disease in patients with critical aortic stenosis undergoing isolated or combined implant valve prosthe...
